1. Transfer Request Form (PDF)

Complete the Transfer Request Form if you are requesting a transfer for any of the following reasons:

1. Overhoused
2. Relocation
3. Medical
4. Safety at Risk
5. Underhoused
6. Non Priority

2. Transfer Arrears Information Sheet (PDF)

The Transfer Arrears Information Sheet explains how rent arrears affect your transfer.

3. Application for Medical Priority (PDF)

Complete the Medical Priority Form if someone in the household has a medical condition as described in the Transfer Policy

4. Application for Safety at Risk Priority (PDF)

Complete the Safety at Risk Priority Form if someone in the household's safety in their current unit or location is considered to be in jeopardy from someone who is not part of household.

5. Tenant Request for Accommodation Form (PDF)

Complete the Tenant Request for Accommodation Form if someone in the household requires a unit that is wheelchair accessible or barrier free or if an additional bedroom is needed for medical equipment.

6. Sharing Bedroom Request (PDF)

Complete the Consent for Sharing Bedrooms if you are a single parent and wish to share a bedroom with your child OR you wish to have your son and daughter share a bedroom.

7. Changes to areas of Preference Form (PDF)

Complete the Areas of Preference Form whenever you want to add or delete locations from an existing transfer request.

8. Caregiver Agency Verification (PDF)

Complete the Caregiver Agency Verification Form if someone in the household requires a live-in caregiver and the caregiver is provided through an Agency.

9. Caregiver Medical Form - Extra Bedroom for Caregiver (PDF)

Complete the Caregiver Medical Form if you require an additional bedroom for the caregiver.

10. Caregiver Verification (PDF)

Complete the Caregiver Verification Form if someone in the household requires a live-in caregiver and the caregiver is contracted by someone in the household.
